Cascades Healthcare Network ACO

Summit Health is now participating in the Centers for Medicare & Medicaid Services (CMS) 2022 Medicare Shared Savings Program (MSSP). Summit Health has launched the Cascades Healthcare Network Accountable Care Organization (ACO) to build on its experience and success as a Next Generation Accountable Care Organization, and its commitment to primary care transformation and population health.

It’s important for patients to know that ACOs do not change any of their Medicare benefits and do not limit their choice of health care providers. ACO patients still have the right to visit any doctor, hospital, or other provider that accepts Medicare at any time, even if that hospital or doctor isn’t a part of an ACO.

If you have questions or concerns, call us at 541-706-6511, or we can talk about them during your visit in our office. You can also call 1-800-MEDICARE or visit Medicare.gov/acos.html.

What is an ACO?

An Accountable Care Organization—or ACO—is a group of doctors, hospitals, and other health care providers that work as a team to give you high-quality, coordinated health care. Learn more about how ACOs can support your health care needs and goals.
