Summit BMC and Kaiser Permanente
Summit BMC is now a direct-contracted provider for Kaiser Permanente members enrolled in Choice Products. Eligible members who live or work in Central Oregon can receive outpatient care at Summit BMC clinics, including:
- Primary care
- Specialty care
- Urgent care
- Routine laboratory services
- Routine Imaging services
Access to services is subject to the terms and conditions of each member’s Kaiser Permanente active plan coverage.
Eligible Kaiser Permanente plans
Members enrolled in the following plans are eligible to receive care at Summit BMC:
- Kaiser Permanente Added Choice® Point-of-Service (POS) plan
- Kaiser Permanente Dual Choice PPO™ plan
- Kaiser Permanente PPO Plus Out-of-Area (OOA) plan
Members enrolled in these plans can visit the Kaiser Permanente NW Choice Products website for details about benefits, care options, pharmacy services, and more.
Members with other Kaiser Permanente plans
Kaiser Permanente members with Traditional medical plans (those not listed above) may receive urgent care services at Summit BMC. Routine and ongoing care should be coordinated through Kaiser Permanente facilities.
Scheduling an appointment
Eligible Kaiser Permanente Choice Product members can schedule care using one of the following methods:
- Schedule online or call Summit BMC at 541-382-4900 to find a provider or book an appointment.
- Contact Kaiser Permanente Customer Service at 1-866-616-0047 and request to panel with a Summit BMC clinic or provider.
