Patient Referrals

First, check to see if your primary care provider needs to see you in order to refer you to a medical specialist. Next, check with your insurance provider to make sure that the physician you’ve been referred to — or wish to see — is on your health plan and that your visit will be covered by insurance. If you are unsure, please give our appointment center a call to see if they can help!

Our friendly representatives will ask for your insurance information so that we can help with any necessary pre-authorizations. After your referral has been approved by your insurance company we will help you make your appointment to see our specialists.

What You Need

Many specialists will need to have the latest information from your Primary Care provider prior to your appointment.

Primary Care Provider

If your Primary Care Provider is a Summit Health Provider, you won’t have to worry as our specialists can view important chart notes from your Primary Care provider directly in your electronic health record. The specialists will also record the details of your appointment in your same medical record so your Primary Care physician can easily see the medical advice you were given by your specialist.

If the Referring Provider is outside of Summit Health, it might take a few days for the Physicians notes to make it to Summit Health and for the Specialist Provider to review those notes. For patients requiring referrals to providers that are outside of Summit Health, we can help process those referrals as well.

Physician & Community Providers

We work with all Physician and community providers and will make sure to help find you the right Physician for your needs.  And if you have a need to go to Portland or beyond, we work with a number of medical providers both regionally and nationally to help ensure our patients get the care you need. Often external referrals require our team to send out relevant information to the Physician we are referring you to in order to have complete care. That process can take a day or two but we will make sure your Physician has the necessary information to provide appropriate care for you.

To make a referral to a Summit Health Provider? It’s as easy as 1-2-3!

1. Download the form and fill out the Referral Request Form or one of your own referral forms.

2. Fax it along with necessary chart notes to 541-318- 3202.

3. The Referral Coordinators will call your patient and book their appointment and do any pre-authorizations that are necessary. If you are referring the patient to multiple specialties we can coordinate all of their appointments to accommodate what is best for the patient.

4. Check your weekly referral log every Friday for an update on the week’s referrals or any pending from previous weeks.

5. The Referral Coordinators follow up with each referral and ensure that all necessary chart notes have been sent back to the referring provider in a timely manner.
