Referrals
When you need to see a specialist, our Referral Department is here to help make the transition smooth and efficient. We help arrange appointments with Summit Health providers as well as external community providers for those patients needing care elsewhere.
First, check to see if your primary care provider needs to see you to refer you to a medical specialist. Next, check with your insurance provider to ensure that the physician you’ve been referred to—or wish to see—is on your health plan and that your visit will be covered by insurance.
Many specialists will need the latest information from your Primary Care provider before your appointment. We will work with your provider’s office to ensure we have all the documentation that is needed before your appointment.

Required documentation for a referral
All departments need the following:
- Demographics
- Insurance information
- Chart notes related to why they are being referred
- Order/referral (and copy of authorization if obtained by an external provider)
- Medication lists
