Referrals

When you need to see a specialist, our Referral Department is here to help make the transition smooth and efficient. We help arrange appointments with Summit Health providers as well as external community providers for those patients needing care elsewhere.

First, check to see if your primary care provider needs to see you to refer you to a medical specialist. Next, check with your insurance provider to ensure that the physician you’ve been referred to—or wish to see—is on your health plan and that your visit will be covered by insurance.

Many specialists will need the latest information from your Primary Care provider before your appointment. We will work with your provider’s office to ensure we have all the documentation that is needed before your appointment.

Required documentation for a referral

All departments need the following:

  • Demographics
  • Insurance information
  • Chart notes related to why they are being referred
  • Order/referral (and copy of authorization if obtained by an external provider)
  • Medication lists

Please note the additional documentation requirements for the following departments.

  • Labs
  • Any diagnostic testing
  • PFT’s
  • CXR
  • Anything pertaining specifically to the problem being referred for

  • Pathology reports (if cancer-related)
  • All lab work

  • Any labs and previous treatment plans
  • Pathology reports

We are happy to partner with you to care for your patient. Our referral department is here to help make it easy and efficient. See the following instructions for more details on the process and the required referral form.

Our goal is to ensure referring providers have all the information necessary to provide the best care possible for their patients. If you have any questions about your patients’ appointments, scheduled visits, or chart notes, please feel free to call our referral department to have your questions answered.

To make a referral to a Summit Health Provider, please:

  1. Download and complete the Referral Request Form (PDF) or use one of your referral forms.
  2. Fax it along with necessary chart notes to 541-706-6474. See the required documents on this page.
  3. The Referral Coordinators will call your patient, book their appointment, and ensure you receive all related medical records in a timely manner. If you are referring the patient to multiple specialties we can coordinate all of their appointments to accommodate what is best for the patient.

Check your weekly referral log every Friday for an update on the week’s referrals or any pending from previous weeks.
