With the move toward health care reform, there is a lot of talk about Patient Centered Medical Home, a model based around proactive, coordinated and preventive care. Bend Memorial Clinic partnered with Clear One Health Plans, now part of Pacific Source, to offer the region’s first patient center medical home. It began in January 2010 with a test panel of 4,000 Clear One Medicare advantage members.
